Ontario Tech acknowledges the lands and people of the Mississaugas of Scugog Island First Nation.

We are thankful to be welcome on these lands in friendship. The lands we are situated on are covered by the Williams Treaties and are the traditional territory of the Mississaugas, a branch of the greater Anishinaabeg Nation, including Algonquin, Ojibway, Odawa and Pottawatomi. These lands remain home to many Indigenous nations and peoples.

We acknowledge this land out of respect for the Indigenous nations who have cared for Turtle Island, also called North America, from before the arrival of settler peoples until this day. Most importantly, we acknowledge that the history of these lands has been tainted by poor treatment and a lack of friendship with the First Nations who call them home.

This history is something we are all affected by because we are all treaty people in Canada. We all have a shared history to reflect on, and each of us is affected by this history in different ways. Our past defines our present, but if we move forward as friends and allies, then it does not have to define our future.

Extenuating circumstances

Sometimes a student may face a serious health/medical situation or uncontrollable extenuating circumstance that warrants special consideration regarding admission, course completion, grades and/or refunds. If this is the case, you are required to complete the form below and attach a letter clearly explaining what you are requesting and why you are requesting it. The letter should be no longer than one page. In addition, to support your request, you must attach one of the following:

  1. If applying because of a medical situation, attach the completed Extenuating Circumstance Request – Health Form or attach a letter from your physician that includes the same information.
  2. Letter of support from a counsellor, clergy member or other acceptable authority.
  3. Copy of death certificate or obituary.

Requests submitted without the appropriate supporting documentation will not be reviewed.


Personal information on this form is collected under the authority of the University of Ontario Institute of Technology Act, SO 2002, c. 8, Sch. O. and will be collected, protected, used, disclosed and retained in compliance with Ontario’s Freedom of Information and Protection of Privacy Act R.S.O. 1990, c. F.31.
