Extenuating circumstances
Sometimes a student may face a serious health/medical situation or uncontrollable extenuating circumstance that warrants special consideration regarding admission, course completion, grades and/or refunds. If this is the case, you are required to complete the form below and attach a letter clearly explaining what you are requesting and why you are requesting it. The letter should be no longer than one page. In addition, to support your request, you must attach one of the following:
- If applying because of a medical situation, attach the completed Extenuating Circumstance Request – Health Form or attach a letter from your physician that includes the same information.
- Letter of support from a counsellor, clergy member or other acceptable authority.
- Copy of death certificate or obituary.
Requests submitted without the appropriate supporting documentation will not be reviewed.
Personal information on this form is collected under the authority of the University of Ontario Institute of Technology Act, SO 2002, c. 8, Sch. O. and will be collected, protected, used, disclosed and retained in compliance with Ontario’s Freedom of Information and Protection of Privacy Act R.S.O. 1990, c. F.31. This information will be used for education, administrative and statistical purposes and to administer registration within the university.
